JOB DESCRIPTION:

 

Summary/Objective

Boston Senior Home Care is a community based organization serving individuals with Long Term Services and Supports (LTSS) needs in Massachusetts. As a Community Partner with multiple Accountable Care Organizations, BSHC provides care coordination to MassHealth recipients with complex long-term medical needs.

PDARNs will be called upon on a per needed basis for a clinical review of comprehensive assessments and long term services and supports (LTSS) care plans conducted and created by BSHC staff. These assessments and care plans will be reviewed on assignment to BSHC CP programs as well as annually.

Essential Functions:

  1. Conducts clinical review and sign off on comprehensive assessment and LTSS Care Plans created by BSHC staff to ensure they are clinically appropriate.      

Competencies:

  • Ability to review comprehensive assessments
  • Knowledge of the biological functions in working with elderly and disabled adults living in a community setting.
  • Considerable knowledge of psychosocial/medical assessment process.
  • Knowledge of the aging process
  • Strong organizational, time-management and priority-setting skills
  • Excellent communication skills
  • Flexibility to accept changing priorities
  • Basic knowledge of community resources and programs
  • Ability to work independently and as part of a team
  • Comprehensive working knowledge of Microsoft applications including Word, PowerPoint, and Outlook.

Work Environment:

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

  • While performing the duties of this job, the employee must be able to carry a 15 to 20 pound load.
  • While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms.

Position Type/Expected Hours of Work:

This is a per diem part-time hourly position. Schedule to be determined and assigned by the program manager.

Travel:

This position has no travel requirements.

 

Required Education and Experience:

Licensed Registered Nurse with extensive knowledge of MassHealth Programs.

 

Additional Eligibility Qualifications:

None required for this position.
