Job Title:                      SCO United Bilingual (English/Cape Verdean) Geriatric Support Service Coordinator (GSSC)

Business Unit:              Senior Care Options Program (SCO)

Department:                 SCO - United Health Care (UHC)

Reports to:                    Program Manager

FLSA:                              Non-exempt

Classification:                Full-time (35 hours per week, Monday-Friday)

Grade:                            B Union

Salary Range:               Starting at $39,072 (commensurate with experience)

New hires who meet both language requirements are eligible for a one-time $1,250 pay differential.

As Geriatric Support Service Coordinator for the SCO Program, you'll work under the general direction of the Program Manager and work collaboratively with other GSSCs.  You will provide initial and ongoing assessments of the SCO member's needs, which includes identifying eligibility for any BSHC services and/or other community resources. You will also develop and coordinate a service plan based on your assessment of each member's needs.  You will work independently and use your ability to multitask in order to conduct home visits, attend family/provider meetings, provide telephone contact with members and others, and document your activities.  You will also work collaboratively within BSHC and externally with the member's formal and informal supports, other SCO representatives, vendors, potential referral sources and other community agencies.  You will also have ample opportunities to actively participate in BSHC's organizational culture.

ESSENTIAL FUNCTIONS

Duties 1-2 are designated as ADA essential functions and must be performed in this job.  All other job duties are secondary functions.

  1. During home visits, conduct multi-dimensional assessments of older adults' functional and social needs incorporating eligibility/need for services, special programs and other community resources.
  2. Provide ongoing assessment of member needs/status through contact with informal supports and with the member via phone and regularly scheduled home visits. Adjust service planning and follow-up as indicated. 
  3. Using a needs assessment while applying your knowledge of resources and problem-solving skills, develop a service plan that incorporates the areas identified within the assessment. Seek supervision for ongoing case management direction, high-risk member issues, vendor needs/issues, etc.
  4. Initiate and follow up with an interdisciplinary consultation and review with SCO personnel for service planning recommendations and to obtain approval of those services. Implement the plan utilizing all available resources, BSHC vendors/programs and other community resources.
  5. Monitor the efficacy and quality of services provided by BSHC through contact with members, their families, and vendors.
  6. Maintain member records by keeping timely documentation, which includes complete and accurate notes, documented communications and required forms.
  7. Provide ongoing advocacy for members, including:
    • Exhibit working knowledge of BSHC services/programs
    • Exhibit working knowledge of other community programs
    • Continually assist in the development of community resources knowledge within BSHC
    • Seek peer or supervisory consultation to explore unfamiliar resource areas
  8. Contribute to BSHC's organizational culture through active participation in team activities, committees, and work groups. Promote a positive working environment through cooperative and respectful interaction with others.
  9. Using all external contacts with vendors and the community at large, continually participate in outreach efforts as a BSHC representative to ensure access to services for all member groups.

SUPERVISORY RESPONSIBILITY

This position has no supervisory responsibilities.

WORK ENVIRONMENT

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, printers, copiers, filing cabinets.

PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

  • The employee must be able to carry 10-15 lbs.
  • The employee is regularly required to talk and hear
  • The employee is frequently required to stand, walk, use hands to finger, handle or feel, and reach with hands and arms

POSITION TYPE/EXPECTED HOURS OF WORK

This is a full-time position, Monday-Friday, 9:00 AM - 5:00 PM.

TRAVEL

This position requires a minimum of up to 80% travel. Must be able to travel under all weather conditions throughout the community to perform home visits.

REQUIRED/PREFERRED EDUCATION AND EXPERIENCE

  • Bachelor's degree required
  • LSW and relevant work experience preferred, but not required

WORK AUTHORIZATION/SECURITY CLEARANCE

  • Must clear CORI background check
