Grade:                          14

Salary Range:              $61, 424.00 - $76,780.00 (commensurate with experience)




 The Senior Manager of Care Management has overall responsibility for ASAP Care Management and Long Term Support Services. Responsibilities include but are not limited to coordination of care, direct care training, planning and directing the work of a group of individuals, monitoring their work and taking corrective action when necessary. This role includes the oversight of two team managers and will report to the Director of Care Management.

Essential Functions:

Essential Job functions: Duties __1_____ through ___14____are designated as ADA Essential Functions and must be performed in this job. All other job duties are secondary functions 

  1. Providing guidance to direct reports comprised of Care Managers, LTSS Coordinators and team managers.
  2. Ensuring clarity around priorities and goals for the entire Care Management and Community Partner program.
  3. Ensure agency compliance with ASAP and ACO program mandates and directives and guides efforts to adjust to changes that impact operations.
  4. Provide consultation to colleagues and community agencies working with the elderly.
  5. Assume a leadership role in agency and community meetings, committees, and task forces.
  6. While supporting the team approach, evaluate the service delivery plan and assist in developing and implementing program policy related to new and existing services.
  7. Providing guidance in hiring and firing requests within his/her group.  
  8. Guiding the talent identification and development processes for a group or function.
  9. Working across functions with peers in other programs to ensure collaboration for shared goals. 
  10. Interacting with Care Management Director for reporting.
  11. Working with Care Management Director and other peers for strategy development and execution planning.
  12. Support a vital, agency wide quality assurance  program that is dedicated to ongoing review of agency policies and practices that reflect the central goal of serving the frail elderly.
  13. Assist the Director in achieving and communicating financial goals.
  14. Facilitating goal-level creation for the broader function and works with team managers to ensure the goals cascade to all workers. 


  1. Strong supervisory skills and an ability to develop personnel as well as manage program.
  2. Flexibility in adjusting to an evolving job role.
  3. Ability to develop innovative programs that respond to the current needs of the health care environment.

Supervisory Responsibility:

This position does have supervisory responsibilities.

 Work Environment:

This position operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets.

Physical Demands:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

  • The employee must be able to carry a 10 to 15 pound load
  • The employee is regularly required to talk or hear.
  • The employee is required to stand, walk, use hands to finger, handle or feel; and reach with hands and arms.

Position Type/Expected Hours of Work:

This is a full time position. Days and hours of work are Monday through Friday 9:00 a.m. to 5 p.m.


Some travel may be required up to 15% travel.

Required Education and Experience: 

  • Master Degrees in Social Work, Psychology, Human Services or health care.
  • Five years experience in a management positions or as a project supervisor.
