Supportive Housing is a program of Boston Senior Home Care (BSHC) that assists low income residents who reside in elderly/disabled housing complexes to age in place and make lifestyle changes that improve the quality of their life.

 The Supportive Housing Resource Specialist (SHRS) assists residents in identifying needs to stay independent in place, and make the appropriate community referrals. The Supportive Housing Resource Specialist works closely with resident services coordinators, on-site medical professionals, hospitals, as well as other health care providers and partnerships, to identify residents in need of Home Care and/or other services, identify and notify any change in status for assigned residents who may require an intervention in order to avoid hospitalization, coordinate any needed ongoing case management to residents who are eligible for services from Boston Senior Home Care, as well as those non-ASAP consumers. The SHRS may also provide assistance with tenancy issues, to help avoid eviction.

Essential Functions

Essential Job functions: Duties ___1____ through ___12____are designated as ADA Essential Functions and must be performed in this job. All other job duties are secondary functions.


  1. Serve as a resource person for the assigned Boston Housing Authority and Connect to Wellness Supportive Housing sites by providing knowledge on consumer and government resources etc. to both ASAP and non-ASAP residents.
  2. Create a schedule in conjunction with the MGH/BSHC Connect to Wellness Team and BHA . Presence at each CTW site (3) would be a maximum of seven hours per assigned day. Presence at each BHA site would be based upon need at any given time.
  3. Work in concert with the CTW Team and BHA property managers to identify residents who seek or require assistance in the form of community referrals and supports to remain safely housed in the community.
  4. Communicate resident issues to existing Case Managers and Geriatric Support Service Coordinators and provide progress and journal entries as required. This includes working in close contact with BostonElder Info and the BSHC Customer Service Unit.
  5. Ability to access the required databases of hospitals, health care providers and partnership to communicate resident issues and provide progress and journal entries to share as required, and to provide requested resident reports every 30 days or no fewer than quarterly.
  6. Develop and maintain positive relationships with Supportive Housing partners
  7. To be trained on Evidence Based classes and conduct trainings to residents.
  8. Assist Program Manager in developing and implementing outreach activities to increase enrollment for all BSHC care management programs.
  9. Foster a sense of community by being an example of tolerance and respectfulness of residents and their property.
  10. Assist Project Coordinator in implementing and teaching direct health and wellness workshops and educational programs. No less than 2 per building over a twelve month rolling period, 6-8 week Evidence Based Trainings (Examples include Tai Chi, Matter of Balance, Chronic Disease Self-Management).
  11. Leverage and outreach to community partners for topical and educational presentations to residents.
  12. Perform other duties as assigned by Program Manager.


  • Must be bilingual in English/Chinese with the ability to speak and write proficiently in both languages.
  • Must possess advanced skills in consumer assessment and be able to assess the physical conditions of the consumer's home as well as the consumer.
  • Interpersonal flexibility, initiative, teamwork, a customer service attitude
  • Excellent oral and written communication skills are key qualifications for this position
  • Solid organizational skills

Supervisory Responsibility

This position has no supervisory responsibilities.

Work Environment

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets.

  • The employee is regularly required to talk or hear.

Position Type/Expected Hours of Work

This is a 35 hours per week full-time position.


This position requires travel to Supportive Housing sites.

Must be able to travel to the community to perform visits at consumers' homes (may have limited accessibility), and travel under all weather conditions.

Required/Preferred Education and Experience

  • BS or BA degree required, licensure preferred.
  • Six months or more years of experience in elder care case management required.
  • Must have computer experience, and be able to perform basic data entry and effectively utilize MS Office Suite.
